Case Managers
You have assessed the injured person’s rehabilitation needs and addressed what is required so they make as good a recovery as possible. You look at every aspect of the person’s physical, intellectual and emotional wellbeing. You make sure the right accommodation, adaptations, aids and equipment are in place and recommend the specialist therapists and other healthcare professionals required for the client.
We will incorporate the agreed, bespoke care plan into the adapted design and layout and integrate the bespoke specialist equipment into the building. Therefore enabling the care plan to be delivered in the most efficient and comfortable way. You can rely on us to be a trusted part of the team.
- When sourcing properties we ensure any potential house can be made fit for purpose and is suitable for adaptation
- We collaborate with you, the wider care team and multiple professionals to find solutions to design
- We listen to the Case Manager and the Healthcare Professionals you appoint to understand how the clinical needs of the client can best be integrated into the design
- We listen to the client and other stakeholders when designing the home and feedback requirements to the care team
- We make sure we understand the clients needs and help shape how their care package could be implemented
- We often ‘drive’ the design to ensure all the above can be accommodated
Specialist Therapists
You may provide physiotherapy, or other specialist therapies such as aquatherapy to the individual client. You may be part of a wider team, usually under the guidance of a Case Manager but not always, you may be engaged by Solicitors or family.
